Cosmetic Procedures Grow Worldwide With Differing Favorites

Reports out of Paris by IMCAS show that the European market for cosmetic procedures is continuing to grow, and is expecting at least another 10% increase this year. Breast Augmentation and liposuction continue to be the top surgical procedures, while anti-wrinkle injections and laser treatments are strongest for non-invasive options.

Cosmetic Surgery Popularity Soars

Asia, too, reports strong growth, with demands highest in eyelid surgery and nose surgery. Growth estimates are upwards of 14% for these procedures, perhaps setting the pace for the region of strongest growth in cosmetic and plastic surgery procedures.

In the United States, growth continues despite the sluggish economic recovery. Some speculate that the continued interest in cosmetic procedures is lead by the need to remain competitive in today's tough job market. Whatever the reason, it certainly appears that cosmetic plastic surgery is no longer considered a taboo topic. We've noticed that more and more patients are openly discussing their surgeries with friends, family and acquaintances, and that it's being talked about as a positive, enhancing life experience.

Men Are Becoming More Open To Male Cosmetic Surgery Procedures

We've noticed an increase in the number of men who are coming in for consultations with Dr. Young. Liposuction, Eyelid Surgery, Breast Reduction, Calf Implants, Tummy Tuck and even Face Lift are more in demand than ever before. While Dr. Young has always had men desiring rhinoplasty (nose job) and liposuction, the increase in these other procedures has noticeably grown over the past several years.

Why the increase? An article just published by Dr. Robert T. Grant in Medscape News, a publication for physicians, points out that, "Although men do not face the same hormonal changes with aging that women do, the male biological clock still ticks, and over time changes in a man's appearance can lead to a negative self-image, which can take a toll on mental and physical health. Today, men are turning to cosmetic surgery in an effort to help break the cycle of interaction between appearance and negative self-image." In other words, men are caring more about their appearance and have realized that cosmetic surgery procedures can make you feel better about yourself. Something women have known for years!

In an ideal world, we'd be able to slow the aging process down if we achieve a good balance diet, exercise, home life, family life, work life, recreation, management of stress, and our health. But for most of us, this is is a difficult if not impossible task to master throughout the course of life. So instead, we strive to create a balance in our aging with use of diet and exercise when we're in our twenties, skin care products in our thirties, use of wrinkle reducers and facial fillers in our forties, more invasive procedures such as skin tightening, fraxel and laser resurfacing in our fifties, and face lifts in our sixties. While women battle with sagging breasts and flabby tummies, men have to deal with receding hairlines, baggy eyes, excessive love handles and man boobs. Face it, aging is not always easy.

Patient's Touching Surgical Story

It's rare when someone reveals themselves openly and honestly. Even more so when it's done publicly. Yet, this is exactly what one of Dr. Young's patients decided to do when she shared her story on an Internet site designed to give helpful information to those interested in breast implants.

"Anonymous" has been making news with her detailed account of why she decided to get breast implants at 49 years of age, and what her experiences were in selecting a surgeon, having the surgery, and her recovery experience. 

We're asked all the time, "What should I expect?", and it's a great question. Brazilian Butt Lift Gaining Popularity in San Antonio

After airing on the Extreme Makeover television show several years ago, the Brazilian Butt Lift has gained in popularity. We've seen a big increase in interest from women in San Antonio.

The Brazilian Butt Lift procedure involves suctioning fat from other areas of the body, especially the hips, and after processing the fat, the fat is injected into the buttock area. The procedure works by narrowing the waist with the liposuction and increasing the buttock fullness.

About 50% of the fat survives and will become a part of the body. The procedure can be repeated several times depending on the ultimate size of the buttock the patient desires. The patients usually must have a moderate amount of fat on them otherwise a buttock implant has to be used. The procedure takes about 1 to 2 hours depending on how much fat is removed and is done as an outpatient. Most patients can return to work in several days.

Other areas patients often select for liposuction are the abdomen, hips, thighs and back, and allow for nice body contouring of the mid to lower body. We are often able to obtain a beautiful hour glass shape where there was excess fat and little definition before. The results can be quite dramatic. Study on Breast Reduction Reveals Age Affects Recovery Outcomes

An interesting study appeared from the Department of Surgery at Johns Hopkins in Baltimore, Maryland concerning whether age affects breast reduction outcome.  17 plastic surgeons were involved in the study which included patients with an average age of 36 years. Age groups were divided into those younger than 40, 40-50 years of age, and those older than 50. The researchers found that the women in the group older than 50 were more likely to experience infection, "with trends towards wound healing problems".  The women in the 40-50 age group also showed a trend toward infection, although not as significant.  A point of interest was that the more aged group did not suffer from any greater degree of more severe complications such as fat necrosis or development of seroma.  The conclusion was that women older than 50 were more likely to develop infection than younger women, and that hormonal deficiency may account for this in part.

What can we learn from a study like this? It could be argued that it is pretty common sense that the older group would experience a higher number of infections. While this may be true, it was interesting that the type of complications were not more severe than those experienced by the younger age groups, and that hormonal deficiency could possibly account for the higher rate in the more aged group. If so, preparation prior to surgery may possibly help beat the odds.

Dr. Young urges all of his patients to take pre-operative surgical vitamins which are designed to boost your immune system and help prepare your body for surgery and the recovery period following.  These are not over-the-counter vitamins, which must be discontinued at least two weeks prior to surgery as some contain ingredients which can impair healing or cause bleeding or other problems.  The surgery vitamins are available to all of our patients, and are taken two weeks before and two weeks following your surgery.  It is important, no matter what your age or health condition, to take all antibiotics and/or other medications prescribed to you prior to and following your surgery as directed.  Followup visits with your surgeon are very important. Anyone experiencing a symptom of infection should call their surgeon's office as early on as possible.

Rhinoplasty Gets Complicated when Septum is Deviated


Rhinoplasty ("nose job") is a cosmetic surgery procedure which can be complicated, and not surprisingly, many plastic surgeons do not enjoy doing them. No two noses are the same, and there can be limitations on what can be done with the nose.  Not everyone can end up with, nor would necessarily want to end up with, a cute, perky little nose with a slight scoop in it. With the nose being in the center of the face, it tends to be put under the microscope by patients when judging results.  And, when presented with common complaints of breathing problems, or a "deviated septum", the task of correcting the exterior shape of the nose can be greatly complicated by the internal issues causing the deviation.

Dr. Robert N. Young is a Board Certified Plastic Surgeon who has been performing Rhinoplasty surgeries for over 20 years, and he has achieved good results when dealing with cosmetic corrections.  When a patient has septal issues however, he and many other qualified plastic surgeons prefer that patients consult with an experienced Ear, Nose & Throat (ENT) physician/surgeon who can assess the internal issues involved. An ENT can diagnose the cause of breathing difficulties, rule out other medical conditions, and prescribe the best course of action for you. In some cases, bone grafts may be needed to correct the problem surgically. The good news is that the treatments are lasting, and outcomes can be very good. 

The golden rule for Rhinoplasty: seek treatment from the professional best suited to help you, depending on if you need cosmetic correction, or correction of a breathing difficulty.

What is the Most Effective Liposuction Technique?

To try and answer this question, the American Society for Aesthetic Plastic Surgery (ASAPS) went to those who perform liposuction most - Board Certified Plastic Surgeons. The professional association's members were surveyed recently, and asked which methods they prefer most, and which ones they perceive as the safest.

The results are in! Overwhelmingly (at 51.4%), they preferred suction-assisted liposuction (or commonly called "traditional" liposuction, which includes tumescent liposuction). Second and third most preferred (23% and 20.9% respectively) were PAL (power-assisted liposuction) and UAL (ultrasound-assisted liposuction).
LAL or "laser-assisted liposuction" accounted for only 3.9% of plastic surgeons preferring this procedure. LAL methods go by many trendy names, but all are assisted by the use of a laser. Many manufacturers claim that the procedure can be performed under local anesthesia, however, in our experience, most patients cannot tolerate liposuction under local anesthesia, especially for large areas. Patient discomfort can lead to the procedure being aborted earlier than desired.

Dr. Young prefers traditional tumescent liposuction techniques and the use of our own PAL "Tickle Lipo". It seems that Dr. Young is not alone in his perception of what works best!

Dr. Young has been performing liposuction in his private cosmetic surgery practice for over twenty years, and has this information to share: When considering your need for fat removal, realize that problem areas are identified by "area", such as abdomen, hips, thighs, or "love-handles" for men! Liposuction is also very effective for arms, neck or chin, and for reducing large breasts on men. When considering liposuction, consult with a Board Certified Plastic Surgeon, as they are the physicians who have received the highest levels of appropriate cosmetic surgery training.

In recent years, Dr. Young has perfected his technique of Brazilian Butt Lift, where fat is suctioned from one or more areas of the body is transferred into the buttocks, creating a shapely, voluptuously full figure that is so in fashion. Fat transfer techniques are also effectively used on the face, where loss of volume naturally occurs as we age. While fat transfer to the breast is once again being talked about as a viable alternative to traditional Breast Augmentation with either silicone or saline breast implants, the use of fat in the breasts often requires multiple procedures to end up with the volume you desire. This is because not all of the fat injected survives. Many prefer the tried-and-true methods that have been perfected in Breast Augmentation with advent of endoscopic technologies which allow for minimal incisions, good placement of implants, and beautiful, lasting outcomes. Liposuction is suitable for both men and women, and Dr. Young has seen a steady rise in the number of men who are electing to have liposuction surgery performed. Keller Funnel Makes Breast Implant Insertion Easier and Less Painful

Having just returned from the cosmetic meeting in Miami, it was exciting to have a first-hand introduction of a new device that makes breast augmentation easier.

The Keller Funnel is a cone shaped device which aides in the placement of silicone breast implants, allowing for smaller incisions and less rubbing as the implant is being inserted. The funnel minimizes trauma to breast tissue, and puts less stress on the implant itself. Because the cone is lined with a special lubricant coating that when moistened makes the funnel extremely slippery, silicone implants (which are pre-filled at the desired size) can easily be inserted without the surgeon ever having to touch the implant. 

What is exciting is that in addition to the standard nipple incision long used for silicone implants, it now allows silicone implants to more easily be inserted through the armpit, which is exciting news for patients who are concerned about scarring on their breasts.  Many of my patients have avoided silicone implants because they did not want incision scars that would be visible on their breasts.  Patients who want no breast scarring often choose the transaxillary incision, which is made in the armpit and leaves a tiny scar that is virtually impossible to see once healed. The transaxillary procedure can be preformed with or without the help of an endoscope (a tube with a small surgical camera on the end). Up to now, saline implants were the primary choice for the armpit incision, because of the distance from the incision to the breast pocket. With the Keller Funnel, silicone implants can more easily be inserted through the armpit.

Even more exciting is that with use of the Keller Funnel, silicone breast implants can be placed  either below the muscle or subfascially.  The subfascial insertion allows for faster recovery times and much less pain, which is the primary post-op complaint.  I am able to achieve outstanding results with an armpit incision using the Keller Funnel and placing the implants below the fascia.  My patients now have even more reason to consider the choice of silicone breast implants with the introduction of this great new invention.

What's New in Breast Implants!

It's been over a year since their re-launch and it appears that the new silicone implants are taking off. The softness, safety and durability of the cohesive gel is attracting a lot of patients to select silicone despite their increased cost. For a little more money than the standard cohesive gel implant, there is also a silicone implant with a saline center (the Becker implant) that can be adjusted after the surgery via a valve under the armpit. When the patient is happy with one size, the valve is then removed.

Many people still feel more comfortable with the saline implants which have been the standard for the past 15 years. All In-Office Surgeries Are Not The Same

Dr. Young finished his general surgery residency at Brooke Army Medical Center in 1980, and went on to serve as Chief of Plastic Surgery, where he performed all types of major reconstructive procedures.

Following Dr. Young's accomplished military career, he went into private practice, founding Aesthetic Enhancement Cosmetic Surgery & Laser Center in 1990. His vision was to bring the most innovative cosmetic surgery procedures to his patients in a comfortable and professional environment. Dr. Young pioneered one of the first in-office Outpatient Surgery Centers which specialized in cosmetic plastic surgery here in San Antonio. His surgical facility is accredited by the AAAASF (American Association for Accreditation of Ambulatory Surgery Facilities), and is designed to provide cost effectiveness for the patient, while allowing total discretion, comfort and safety.

Dr. Young has limited his practice to cosmetic surgery to concentrate his skills on the aesthetic improvement of his patients' beauty. Incorporating the latest technological procedures, Dr. Young specializes in endoscopic (minimal incision) axillary breast augmentation, and remains a leader in the field of liposuction. Providing cosmetic surgery for total body enhancement, he offers all types of breast surgery, tummy tuck, body sculpting, and facial surgeries such as face lift, eyelid surgery and laser resurfacing with state-of-the art lasers including the Sciton.

Dr. Young is an accomplished athlete, having competed in many local and national tri-athlete competitions, marathons and biking events. When not working, training or competing, he spends time at home with his wife, three wonderful dogs and two cats.
