Man Arrested For Injecting Dangerous Silicone Into People's Buttocks For Buttock Enhancement

Fat injections to the buttocks has become very popular over the past several years. There are basically two different ways to achieve fuller buttocks - buttock implants or a procedure called the Brazilian Butt Lift. For the Brazilian Butt Lift, fat is removed from unwanted areas by liposuction. After the fat is filtered it is injected into the buttocks, adding contour, shape and the fullness desired. There are also unscrupulous impostors offering unsafe silicone injections.

Today, a story has surfaced about the latest con who has used this method to perform "enhancement" procedures on unsuspecting victims. The story comes out of Palm Beach, Florida where two victims, a male and a female, brought light to the illegal injections being performed by an unlicensed man.

The injections were being performed by the arrested suspect injecting silicone into the buttocks and closing the incisions with Krazy Glue. He was reportedly charging $200 per injection of a silicone substance not approved for use in the body and one that can cause serious harm. Both victims became known following their being admitted to the hospital for "severe infection, pain, bleeding, oozing wounds, and disfigurement," according to WPTV-TV.

Breast Implant Size Best Determined By Patient

Selecting the "right size" breast implant is a very personal decision when considering breast augmentation, and is one that many women agonize over. No one wants to feel that they've chosen a size that is too small or too big. So what is the best way to decide?

One thing we hear a lot is, "My friend looks great and she got 425cc implants. That's the size I want." Others, after reading materials on-line are influenced by what is perceived as the right size by others. Some have no idea where to start.

We find it helpful to let our patients explore a range of sizes by trying on "sizers", which are breast shaped firm silicone. Should My Plastic Surgeon Be Board Certified?

There's good reason to check whether your plastic surgeon is Board Certified. Too often, news emerges with stories of patients who elect to go to providers who may not have the qualifications to perform cosmetic procedures or surgeries. With the popularity of cosmetic plastic surgery, it can attract unqualified (and sometimes unscrupulous) people who are lured by what they see as quick profit.

In some cases, the service provider may not even be a legitimate doctor. There have been several stories this past year about people posing as doctors and performing non-surgical procedures as well as actual surgeries in hotel rooms or back offices. There have been cases of people being injected with non-medical ingredients such as cement or silicone directly into the breasts, buttocks and even the face. These are highly toxic and can be extremely caustic, causing burns, infection, serious wounds and even disfigurement. Botox® and facial fillers are being used by people who have no training in facial nerve structure or the underlying muscles of the face, and mis-use of these products can result in droopy eyes, temporary paralysis, and even nerve damage.

In other cases, it may be a doctor who does not have sufficient training in this specialized field. They may be a generalist, gynecologist or other physician who has no formal training or experience in plastic surgery. They may have simply taken a weekend course in breast augmentation or liposuction and may have little experience with surgery. Generally people undergoing any type of surgery want someone with specialized training and competence.

Some are turning to the trend in "awake" surgeries. Procedures performed while the patient is "awake" can be risky if they do not have the equipment necessary to monitor a patient, do not have emergency equipment necessary to resuscitate a patient should there be a complication or if a licensed anesthesiologist is not in attendance during the surgical procedure. Even though the patient may be "awake", it does not decrease the risk associated with the sedation medications that patients are given. It is still surgery!

Tourist or vacation plastic surgery is also seeing an increase as those who want cosmetic surgery but don't want to or cannot pay the going rate seek alternatives. We've seen women who have flown to a foreign country such as Mexico, Asia or South America to receive a $2,000 tummy tuck and now have complications such as severe infection or a poor outcome that could cost many more thousands to try to correct. It is rare to receive appropriate pre-operative and post-operative care at foreign facilities, with patients eager to get on with the vacation end of their trip, or in getting back home, putting themselves at even greater risk of complications. It is usually recommended that you do not fly for at least 2 weeks or more following any type of surgery, and cosmetic plastic surgery is no exception.

So what is the difference between an internist and a plastic surgeon?

Obtaining a medical license to diagnose and treat patients means a physician meets a minimum level of competency requirements to become a doctor, but it is not specific to any one specialty. Board Certification demonstrates a level of expertise in a specialty and is considered the "Gold Standard". Board Certification by the ABPS ensures that the surgeon has had at least six years of surgical training, years of surgical experience, has passed both oral and written exams and has had surgical cases reviewed by the Board in order to demonstrate proficiency within the field of plastic surgery. They certify that they only operate in accredited medical facilities such as hospitals and accredited outpatient surgery centers. They are licensed physicians who have made a choice to make plastic surgery their primary focus as a medical professional.

New Bra Shows Promise In Helping To Detect Early Breast Cancers

News came out this week about a new form of breast cancer detection that is creating quite a bit of optimism and hope for the advancement of early detection. As October is Breast Cancer Awareness Month, we're excited to bring you news of a new bra that can be used to help detect breast cancer. It is reported that it has been able to detect breast cancers in very early stages which would not normally be detected by conventional methods now in use.

Currently in the trial stage, the bra's developer, Lifeline Biotechnologies, has already performed three trials which have demonstrated that the bra detects abnormalities with at least 90% accuracy in women of all ages. The bra, called First Warning Systems, is easy to use, and is said to look and feel like a sports bra. A woman wears the bra for a set period of time during which the sensors which are encased throughout the bra, detect and record small changes in temperature in breast tissue. After collection, the data is transmitted via the Internet, analyzed and the results are then sent to the woman's personal device.



According to the company's website, "As tissues transition from normal to hyperplasia, to atypical hyperplasia, to cancer in-situ to, to invasive cancer they develop their own distinct thermal finger print and can be compared with normal tissue temperature." They claim that they produce far fewer false positives, which with other detection methods can be as high as 50-60%.  The reliability of mammograms has come into question because of their high rate of false positives. This can be traumatic, and costly, to women. If trials work out well, early detection may be as simple as wearing a bra in the comfort and privacy of your own home.

We will continue to monitor the results of the continued testing, and will update this promising technology. In the meantime, Dr. Robert Young urges women to follow current recommendations for routine office visits, self breast exams, and mammography.

L'Oreal Receives Warning From FDA

Last month, the FDA sent a warning to cosmetic giant L'Oreal to stop advertising skin care products with language that makes the products sound like they are providing the same benefit as prescription products (drugs). A September 11, 2012 Reuters article detailed several L'Oreal products which have come under scrutiny including: Genifique Repair Youth Activating Night Cream and Absolute Eye Precious Cells Advancing Regenerating and Reconstructing Eye Cream.

The FDA will go so far as to seize products and issue injunctions against manufacturers and distributors when claims are made that products act as drugs.

Lancome USA, a L'Oreal subsidiary, reportedly crossed the line with advertising on it's website. One actual on-line ad read, "You sleep. Genes don't. Wake up to smooth, fresh, visibly repaired skin. During the night, the speed of cell regeneration doubles. By screening over 4,000 genes, Lancôme's laboratories identified the genes responsible for making this activity happen and used their discoveries to create nighttime skincare that boosts the activity of genes². Night after night, skin is appears younger and more rested, as if you had slept at least two extra hours.¹"

This type of advertising violates FDA rules.

Other claims on products have included skin creams which could "boost the activity of genes" or "stimulate cell regeneration" to reduce signs of aging. According to the FDA, "any product that is intended to affect the structure or function of the human body is classified as a drug." The product must undergo testing and approval from the FDA before these types of claims can be made. The process is rigorous and involved.

Many of the products we sell at our Medical Day Spa are prescription products that have been cleared by the FDA for sale as medical grade products. Yet we notice products all the time in magazines, non-physician offices, stores, and on-line that imply that products offered are as effective as prescription skin care products. It is difficult for most consumers to know the difference. In many women's magazines, you can see prescription items reviewed right next to products that have ingredients with far less effectiveness. Many consumers do not know the differences between Retin A and retinoid complexes for example. The names sound similar, and implies the same type of benefit, but they are not equivalent in effectiveness. It is also important to note that prescription-strength products cannot legally be sold on-line without a prescription.

When a skin care professional works under the guidance and supervision of a physician you have access to products which have been approved both in effectiveness and safety by the FDA.

Men Are Becoming More Open To Male Cosmetic Surgery Procedures

We've noticed an increase in the number of men who are coming in for consultations with Dr. Young. Liposuction, Eyelid Surgery, Breast Reduction, Calf Implants, Tummy Tuck and even Face Lift are more in demand than ever before. While Dr. Young has always had men desiring rhinoplasty (nose job) and liposuction, the increase in these other procedures has noticeably grown over the past several years.

Why the increase? An article just published by Dr. Robert T. Grant in Medscape News, a publication for physicians, points out that, "Although men do not face the same hormonal changes with aging that women do, the male biological clock still ticks, and over time changes in a man's appearance can lead to a negative self-image, which can take a toll on mental and physical health. In an ideal world, we'd be able to slow the aging process down if we achieve a good balance diet, exercise, home life, family life, work life, recreation, management of stress, and our health. But for most of us, this is is a difficult if not impossible task to master throughout the course of life. So instead, we strive to create a balance in our aging with use of diet and exercise when we're in our twenties, skin care products in our thirties, use of wrinkle reducers and facial fillers in our forties, more invasive procedures such as skin tightening, fraxel and laser resurfacing in our fifties, and face lifts in our sixties. While women battle with sagging breasts and flabby tummies, men have to deal with receding hairlines, baggy eyes, excessive love handles and man boobs. Face it, aging is not always easy.

Brazilian Butt Lift Gaining Popularity in San Antonio

After airing on the Extreme Makeover television show several years ago, the Brazilian Butt Lift has gained in popularity. We've seen a big increase in interest from women in San Antonio.

The Brazilian Butt Lift procedure involves suctioning fat from other areas of the body, especially the hips, and after processing the fat, the fat is injected into the buttock area. The procedure works by narrowing the waist with the liposuction and increasing the buttock fullness.

Followup on PIP Breast Implants

On 12-16-11, we ran a story about the breast implants used in France.  The latest news is that the head of the company and at least one of his executives have been arrested.  The company, Poly Implant Prothese (PIP) has since ceased operations, but this has not stopped authorities from seeking manslaughter charges against those reponsible for using substandard silicone in breast implants which were distributed mainly in Europe and South America.

As we previously reported, the USA manufacturers Dr. Young obtains his silicone breast implants from are medically sterilized and safe, and are made from a medical grade silicone. The majority of breast implants we use are Mentor's MemoryGel silicone cohesive gel-filled implants which are made here in the United States under strict FDA guidelines and inspections. Men Now On-Board With Botox®

As reported on Good Morning America this morning (Monday, 01-09-2012), men have joined the ranks of those requesting and receiving Botox® for facial rejuvenation!  Many men, probably a good deal of whom were urged by women in their life, are seeking Botox® to ease the fine lines and wrinkles that appear as we age.

Botox® has long been used by women for treatment of forehead frown lines and wrinkles around the eyes (usually called crows feet).  It's popularity has made it one of the most frequently asked for non-surgical treatments in most plastic surgeon offices.  Properly administered, the effects can significantly reduce wrinkles and take years off the face. We're happy to see that men are removing the stigma of using plastic surgeon services to give them an edge in the workplace, and feel good about themselves in general.

Are Adjustable Breast Implants A Good Option?

We receive a substantial number of calls about "adjustable breast implants".  Dr. Robert N. Young does do the adjustable breast implants and has always had them, but they may not be the best choice for everyone.  Many people think that this is a new type of implant that is somehow superior to traditional saline or silicone breast implants, but let's look at what they are, as well as the pros and cons of selecting them.

Adjustable breast implants originated in the mid 1980's by Becker, in conjunction with the breast implant manufacturer Mentor.  They were developed as a tissue expander used in breast reconstruction surgeries following mastectomy.  By the late 1980's, surgeons began using the Becker tissue expander in breast augmentation. It was thought of as a solution to achieve the ideal size. While the Becker expanders are still available, many no longer find it suitable due to cost and FDA restrictions.  A new implant called the Spectrum implant, also available by Mentor, is a permanent saline expander that uses the same Becker fill system.

According to Mentor, their "innovative line of SPECTRUM® Breast Implants makes the decision-making process easier because the size can be adjusted after surgery."  These implants allow the surgeon to adjust the size of your implant for up to six months after your procedure.

The ability to go back and change the size to be larger or smaller at a later time is seen as a plus for some. After inserting the implants, the surgeon leaves the filling tube between the stitches of the closed incision and within 3-6 months following the initial surgery (after the swelling has subsided), the surgeon is able to go back and change the size of the implants by taking saline out or putting more in. After the 6 months, the port is removed by the surgeon, and you can no longer change the size of the implants.

The actual Breast Augmentation surgery with adjustable implants is the same as when you use standard saline or silicone implants. The adjustable implants are more expensive, and with Dr. Young you must still undergo general anesthesia, so it can be a more expensive option. You are also limited in the amount of saline that you are able to put in or take out depending on the size of the implant that was chosen from the beginning. This is true whether you use the adjustable implants or standard saline breast implants. In using standard saline implants, we still have the option of adjusting the size up or down during the surgery, and adjustments can be made any time in the future through the fill port with standard implants. Unlike standard implants, with the "adjustable" implants, once the special filling tube is removed, you can never use it again to remove or add saline.

Be careful of media hype about adjustable implants, TUBA breast augmentation (transumbilical), and other promotions that are not always the best solution. Even though adjustable breast implants may be suitable for one woman, it is not the best answer for all. Read more about Breast Augmentation surgery.

Study on Breast Reduction Reveals Age Affects Recovery Outcomes

An interesting study appeared from the Department of Surgery at Johns Hopkins in Baltimore, Maryland concerning whether age affects breast reduction outcome.  17 plastic surgeons were involved in the study which included patients with an average age of 36 years. Age groups were divided into those younger than 40, 40-50 years of age, and those older than 50. The researchers found that the women in the group older than 50 were more likely to experience infection, "with trends towards wound healing problems".  The women in the 40-50 age group also showed a trend toward infection, although not as significant.  A point of interest was that the more aged group did not suffer from any greater degree of more severe complications such as fat necrosis or development of seroma.  The conclusion was that women older than 50 were more likely to develop infection than younger women, and that hormonal deficiency may account for this in part.

What can we learn from a study like this? It could be argued that it is pretty common sense that the older group would experience a higher number of infections. While this may be true, it was interesting that the type of complications were not more severe than those experienced by the younger age groups, and that hormonal deficiency could possibly account for the higher rate in the more aged group. If so, preparation prior to surgery may possibly help beat the odds.

Dr. Young urges all of his patients to take pre-operative surgical vitamins which are designed to boost your immune system and help prepare your body for surgery and the recovery period following.  These are not over-the-counter vitamins, which must be discontinued at least two weeks prior to surgery as some contain ingredients which can impair healing or cause bleeding or other problems.  The surgery vitamins are available to all of our patients, and are taken two weeks before and two weeks following your surgery.  It is important, no matter what your age or health condition, to take all antibiotics and/or other medications prescribed to you prior to and following your surgery as directed.  Followup visits with your surgeon are very important. Anyone experiencing a symptom of infection should call their surgeon's office as early on as possible.

You may have noticed an article which has hit the web on France's scandal involving silicone breast implants.  The huffingtonpost.com reported that more than 30,000 women in Europe have received silicone implants which were produced by the French implant company Poly Implant Prosthesis (PIP) using industrial grade silicone rather than the medical grade silicone which is used here in the United States.

Heed Your Surgeon's Advice on Exercise

A bizarre article caught my eye this morning about a woman whose breast implant disappeared (the article actually described it as "her body 'swallowed' one of her implants") while she performed a Pilates stretching exercise.  Thinking that this sounded a little strange, I read the article, which appeared on the site TheBodyOdd on msnbc.com a couple of days ago, and was published online in the New England Journal of Medicine.

Apparently, the woman had several surgeries performed in the past. She was a breast cancer survivor and had a double mastectomy.  She also had a recent surgery to repair her heart's mitral valve, which involved separating some of the muscles that run between the ribs.  It is thought that when the woman attempted the Pilates move, called the Valsalva maneuver, the muscles in her chest contracted and may have pushed the implant through her ribs and into the space between the lungs.  It is felt that this area was fragile after her multiple surgeries, and that tissue was more easily torn.  It has been described as a "freak occurrence".  To view the article: http://bodyodd.msnbc.msn.com/_news/2011/12/14/9444713-womans-breast-implant-disappears-during-pilates.

The good news is that the woman is fine.  Surgeons found the implant and placed it back where it belongs.
But what does all this mean for the the typical women who is considering or already has breast implants? 

First, realize that when stories like this appear, they truly are rare and unusual occurrences. Second, while the article does not mention how long ago the woman had her breast implant surgery, it is a reminder to listen to and FOLLOW the advice you receive from your surgeon. Some complications can be avoided by following this advice. Your Board Certified Plastic Surgeon will give you a time frame in which you are to avoid all exercise, then tell you how soon you can return to moderate exercise, and when you can return to your regular full workouts. The same can apply for your recovery time and when it is safe to return to work, lifting, housecleaning, and even sex. It is easy to return too early because you feel so good, but complications CAN occur if you push the limits too early, or if you "overdo" it.  If there is any doubt, please call your surgeon and ask.

Rhinoplasty Gets Complicated when Septum is Deviated

Rhinoplasty ("nose job") is a cosmetic surgery procedure which can be complicated, and not surprisingly, many plastic surgeons do not enjoy doing them. No two noses are the same, and there can be limitations on what can be done with the nose.  Not everyone can end up with, nor would necessarily want to end up with, a cute, perky little nose with a slight scoop in it. With the nose being in the center of the face, it tends to be put under the microscope by patients when judging results.  And, when presented with common complaints of breathing problems, or a "deviated septum", the task of correcting the exterior shape of the nose can be greatly complicated by the internal issues causing the deviation.

Dr. Robert N. Young is a Board Certified Plastic Surgeon who has been performing Rhinoplasty surgeries for over 20 years, and he has achieved good results when dealing with cosmetic corrections.  When a patient has septal issues however, he and many other qualified plastic surgeons prefer that patients consult with an experienced Ear, Nose & Throat (ENT) physician/surgeon who can assess the internal issues involved. An ENT can diagnose the cause of breathing difficulties, rule out other medical conditions, and prescribe the best course of action for you. In some cases, bone grafts may be needed to correct the problem surgically. The good news is that the treatments are lasting, and outcomes can be very good. 

The golden rule for Rhinoplasty: seek treatment from the professional best suited to help you, depending on if you need cosmetic correction, or correction of a breathing difficulty.

Is Dissatisfaction With Your Body a Problem?

I came across an interesting article published recently in a professional journal discussing the possibility that people who are very unhappy about the size or shape of their nose could mean that they suffer from a psychiatric disorder called Body Dysmorphic Disorder.  A trained and Board Certified Plastic Surgeon is trained to look for patients who may suffer from Body Dysmorphic Disorder (BDD) for a variety of reasons.  Those with severe BDD can have an inaccurate perception of the way their body actual appears, and this can disrupt their everyday quality of life.  The most common conditions associated with severe BDD is eating disorders such as Bulemia or Anorexia, where the patient may actually see themselves as "fat", when in reality they are extremely thin or emaciated. BDD could conceivably make one obsess about their nose, their legs, breasts, stomach, or other body area to the point where no amount of "correction" would prove satisfying.

Cosmetic Plastic Surgery can provide an increase in one's self esteem, help clothes fit better, and give one confidence. For those with BDD, cosmetic surgery usually does not "solve" the issues they have with their body, and they may be more apt to seek out revisional surgeries to the same area. Making sure that a patient has reasonable and obtainable expectations is key to successful outcomes in cosmetic plastic surgery, and most cosmetic surgeons want to know so they can deliver the results you want. Be sure to discuss your expectations clearly with the Board Certified Plastic Surgeons you consult with, and listen carefully if they advise that the surgery may not provide quite what you expect. No one wants to find themselves caught in an endless cycle of revisional surgery, whether you have had rhinoplasty, liposuction or breast augmentation.

On a good note, we find that patients who have taken time to research their procedure and their surgeon are often the most satisfied.  While the internet can be a great tool for information, not all information is accurate, so ask questions from your surgeon if something doesn't sound right, or if there is something of particular importance to you.  Take time to think over your reasons for wanting the surgery, and what you feel it will do for you.  If you feel your expectations are reasonable, then plastic surgery can quite possibly be a good choice for you.

Use Breast Implants From Reputable Manufacturers

Another story which appeared in the news today about breast implants sounds shocking on the surface. While this condition may exist, and rupture and other problems can be more prevalent in larger implants, in the United States, the leading manufacturers of saline and silicone breast implants provide implants in sizes that have proven to be safe. These are strictly regulated.  In addition, Board Certified Plastic Surgeons who specialize in Breast Augmentations receive training in the appropriate use and sizing of breast implants.

The very large sizes mentioned in the article (G-Cup, FF-Cup etc) are no longer used in this country.  We also have very strict manufacturing standards which must be met in making breast implants, and there have been many advances over the years to ensure safety. In our experience, most women select implants from 350cc up to 800cc, which can increase your breast size from one to a few bra cup sizes.  Most women don't want to look abnormal, clownish or "over-the-top".  Keeping in proportion to the rest of your body will almost always give the most pleasing result.

We recommend that if you are considering breast implants, go to a Board Certified Plastic Surgeon who is trained and experienced in Breast Augmentation, and who uses quality saline or silicone gel implants.  He or she will be able to advise you on the risks associated with the type and size implant you desire, and will have your health, safety and goals in mind when helping with this important decision.

Choose Credentialled, Qualified and Experienced Specialists

Stories pop up over and over again about patients who willingly select doctors without appropriate credentials, or those who practice out of uncertified facilities. The outcomes of these are often tragic for the patients involved.  What sometimes saves a bit of money ends up putting their health or life in jeopardy.

The most recent story appeared this week about a woman who received cheek injections which turned out to be a mixture of ingredients including concrete!  Another patient who went to the same "doctor" sealed her incisions with superglue!  Unqualified doctors have performed procedures from Breast Augmentation to Brazilian Butt Lifts using products which are not medical grade, nor approved for use in cosmetic surgery.  Others perform "awake" procedures under local anesthesia in their office.  These can be very dangerous.

The woman "doctor" in question was not Board Certified, was not a plastic surgeon, nor a licensed physician of any kind.  She did not perform the surgeries at a hospital or in a certified or licensed outpatient surgery center.

What is the Most Effective Liposuction Technique?

To try and answer this question, the American Society for Aesthetic Plastic Surgery (ASAPS) went to those who perform liposuction most - Board Certified Plastic Surgeons. The professional association's members were surveyed recently, and asked which methods they prefer most, and which ones they perceive as the safest.

The results are in! Overwhelmingly (at 51.4%), they preferred suction-assisted liposuction (or commonly called "traditional" liposuction, which includes tumescent liposuction). Second and third most preferred (23% and 20.9% respectively) were PAL (power-assisted liposuction) and UAL (ultrasound-assisted liposuction).
LAL or "laser-assisted liposuction" accounted for only 3.9% of plastic surgeons preferring this procedure. LAL methods go by many trendy names, but all are assisted by the use of a laser. Many manufacturers claim that the procedure can be performed under local anesthesia, however, in our experience, most patients cannot tolerate liposuction under local anesthesia, especially for large areas. Patient discomfort can lead to the procedure being aborted earlier than desired.

Dr. Young prefers traditional tumescent liposuction techniques and the use of our own PAL "Tickle Lipo". It seems that Dr. Young is not alone in his perception of what works best!

Dr. Young has been performing liposuction in his private cosmetic surgery practice for over twenty years, and has this information to share: When considering your need for fat removal, realize that problem areas are identified by "area", such as abdomen, hips, thighs, or "love-handles" for men! Liposuction is also very effective for arms, neck or chin, and for reducing large breasts on men. When considering liposuction, consult with a Board Certified Plastic Surgeon, as they are the physicians who have received the highest levels of appropriate cosmetic surgery training.

In recent years, Dr. Young has perfected his technique of Brazilian Butt Lift, where fat is suctioned from one or more areas of the body is transferred into the buttocks, creating a shapely, voluptuously full figure that is so in fashion. Fat transfer techniques are also effectively used on the face, where loss of volume naturally occurs as we age. While fat transfer to the breast is once again being talked about as a viable alternative to traditional Breast Augmentation with either silicone or saline breast implants, the use of fat in the breasts often requires multiple procedures to end up with the volume you desire. This is because not all of the fat injected survives. Many prefer the tried-and-true methods that have been perfected in Breast Augmentation with advent of endoscopic technologies which allow for minimal incisions, good placement of implants, and beautiful, lasting outcomes. Dr Young's Thoughts on Awake Surgery

Local anesthesia for surgical procedures is not always the best choice. Often times the operating physician is giving sedation medication and using local anesthesia without an Anesthesiologist in order to save money. I think it is much safer to use an Anesthesiologist to give the patient the proper level of comfort during the procedure, as well as to monitor their level of consciousness. If you are feeling pain during the procedure the physician will often stop short of getting you a successful result. A few hundred dollars more for a Board Certified Anesthesiologist is definitely worth the safety and success of a several thousand dollar procedure.

Keller Funnel Makes Breast Implant Insertion Easier and Less Painful

Having just returned from the cosmetic meeting in Miami, it was exciting to have a first-hand introduction of a new device that makes breast augmentation easier.

The Keller Funnel is a cone shaped device which aides in the placement of silicone breast implants, allowing for smaller incisions and less rubbing as the implant is being inserted. The funnel minimizes trauma to breast tissue, and puts less stress on the implant itself. Because the cone is lined with a special lubricant coating that when moistened makes the funnel extremely slippery, silicone implants (which are pre-filled at the desired size) can easily be inserted without the surgeon ever having to touch the implant. 

What is exciting is that in addition to the standard nipple incision long used for silicone implants, it now allows silicone implants to more easily be inserted through the armpit, which is exciting news for patients who are concerned about scarring on their breasts.  Many of my patients have avoided silicone implants because they did not want incision scars that would be visible on their breasts.  Patients who want no breast scarring often choose the transaxillary incision, which is made in the armpit and leaves a tiny scar that is virtually impossible to see once healed. The transaxillary procedure can be preformed with or without the help of an endoscope (a tube with a small surgical camera on the end). Up to now, saline implants were the primary choice for the armpit incision, because of the distance from the incision to the breast pocket. With the Keller Funnel, silicone implants can more easily be inserted through the armpit.

Even more exciting is that with use of the Keller Funnel, silicone breast implants can be placed  either below the muscle or subfascially.  The subfascial insertion allows for faster recovery times and much less pain, which is the primary post-op complaint.  I am able to achieve outstanding results with an armpit incision using the Keller Funnel and placing the implants below the fascia.  My patients now have even more reason to consider the choice of silicone breast implants with the introduction of this great new invention.

More Men Looking At Cosmetic Surgery

I've noticed more men coming to my specialty practice for cosmetic surgery procedures over the last year or two. What is most striking is that in addition to the more common liposuction and rhinoplasty, there has been an increase in full facelift, abdominoplasty "tummy tuck" and blepharoplasty "eyelid lift". I am also seeing requests for calf implants, pectoral "pec" implants, and other procedures which promote a more healthy, youthful appearance.

Many articles suggest that both men and women are trying to "put their best foot forward" in terms of remaining competitive in the current job market, and the fact that the number of men coming in for Botox(R) and facial fillers may support this theory.

Male cosmetic plastic surgery seems to be growing in popularity and I am happy to discuss both surgical and non-surgical alternatives to help men achieve their goals.

Board Certified Plastic Surgeon

We've seen a new trend in Cosmetic Plastic Surgery...promotions of "Awake Breast Augmentations".  It is talked about as something new, but in actuality, surgeries performed under "local anesthesia", or "twilight anesthesia" have been around for many years.  Whether it is a good choice for patients is being hotly debated.

Because Breast Augmentation has become such a popular procedure, and is more readily talked about by society, some feel it has become somewhat frivolized.  It is easy to forget that this is still a major surgical procedure. While endoscopic (minimal incision) techniques have made the surgery less invasive and easier to recover from, the procedure is best done under general anesthesia with a Board Certified Anesthesiologist. "Twilight anesthesia" can be dangerous without an anesthesiologist present.

There is quite a bit of debate recently about practitioners who are advertising "awake" surgeries.  These are usually performed by physicians who are not Board Certified Plastic Surgeons, and may not even be Plastic Surgeons at all.  Sometimes they are Dermatologists, General Surgeons, or even Family Practitioners who elect to perform the surgery under local anesthesia because they do not have  privileges in a hospital or certified surgery center to perform those procedures. In my Surgery Center, all anesthesia is administered and monitored by Board Certified Anesthesiologists who are experienced in Cosmetic Plastic Surgery.  I feel this is best for my patients, and myself, as it allows me to concentrate on my area of expertise, which is the surgery itself.

My Surgery Center is fully certified by the AAAASF and meets strict standards to perform surgical procedures. Sure, the surgery costs a little more, but it is worth having the extra standards and safety factors. That's why doctors are credentialed and surgery centers are certified. If we didn't adhere to these standards we would be cheaper also. So when you get a fee quote that is cheaper because it is under local anesthesia in an office back room, be careful of what you are asking for.

Board certification by an ABMS member board provides assurance of a physician’s expertise in a particular specialty and/or subspecialty of practice. Professional organizations, such as the American Society of Plastic Surgeons (ASPS), the American Society for Aesthetic Plastic Surgery (ASAPS) require that members be Board Certified within their specialty.

Some advertisements even go so far as to state that you'll be awake enough during your procedure to pick the size of your breast implants (this would only be possible with saline implants which are inserted and then filled to the desired size) during your surgical procedure itself!  If you have had sedation you are going to feel woozy and wobbly.  I don't think you would be coherent enough to make this selection during surgery!

We recommend that you ask questions that will ensure that you are well informed concerning the qualifications and experience of your surgeon and the types of anesthesia that you are offered.  Here are questions generally recommended:

• Are you Board Certified? 
• If so, what specialty board?
• Are you licensed to practice surgery in this state?
• How many breast augmentation surgeries do you perform each year?
• How many years have you performed breast implant procedures?
• Where is my procedure performed?  
• Is the surgical facility accredited?
  
• Who administers anesthesia during my surgery?
• Is the person who administers my anesthesia Board Certified in this specialty? 
  
• What are my options for anesthesia?
