Important Breast Implant Update

The old saying that there is more than one way to skin a cat is absolutely true in terms of breast augmentation surgery.

When you visit a physician to discuss your breast augmentation, beware of the physician that claims his way is the only or “best” way to do the surgery. There are numerous options in breast implants that will provide excellent results.

First of all there are two types of breast implants, saline and silicone. Both are FDA approved implants and each implant has its features. 

The silicone is cohesive in order to minimize the leakage and it appears to have a more natural feel. With silicone gel implants, you are able to select different size implants for each breast in order to create a balanced look, but silicone implants come pre-filled and the sizes cannot be changed without removal and replacement.

The saline implant is filled with a sterile saline solution. The implant has a watery feel but is adjustable since there is a valve in it. This allows the surgeon to adjust the implant during surgery in case you have a difference in the size of your breasts, and can have small adjustments made afterwards if desired. The saline implant will deflate when it leaks. The silicone will not. 

There are different projections of implants, low, moderate or high profile. I prefer the high profile to obtain more projection and less width. Again, both implants are excellent choices and your surgeon should offer both options. Any surgeon that claims one type of implant is better than the other is not being truthful.

There are also many ways to insert the implant. My personal favorite is axillary, where the scar is hidden in the armpit. If the patient prefers, I will make the incision around the nipple (periareolar) or below in the breast fold (inframammary). These are the three most common ways to insert the implant. Once again, the most popular choices are: axillary, periareolar, or inframammary. There is no right or wrong way.

The only other option is whether you put the implant above or below the muscle. Many physicians put it below the muscle, but in certain instances, you can put it above the muscle, particularly with silicone implants. If you have excessive skin and some sagging, placing it above the muscle seems to fill it out better. If you have less tissue, going under the muscle seems to blend the implants to the breast better, diminishing the “ball” effect.

The bottom line is that there are many ways to insert a breast implant and also two basic types of implants. All physicians have a favorite way, but all approaches are proven to be successful. It’s like buying a new car, if the salesman offers you only one car as an option to buy, you wouldn’t be happy. You want to see more cars and options or you are probably going elsewhere. It’s the same for breast implants. Get all the options from your physician. If he or she insists on only one, be sure to get another opinion.

You should always have a say in these options in order to select what is most important or best for you.  The choice of size should always be yours. You should be able to select sizes that provide the look you prefer, whether athletic, natural or large. Some women want to look like they have breast implants, and others prefer a more natural look.
